Service Profile: Transition Care Program
Transition Care is a short-term Program that aims to help older people to return home and maintain independent living after a hospital stay.
TCP is available through Rochester and Elmore District Health Service to eligible patients.
It covers services providing low intensity therapy such as physiotherapy and occupational therapy, as well as social work, nursing support or personal care to conserve and improve physical and/or cognitive functioning.
REDHS can support TCP in either an inpatient and home-based setting, depending on an individual’s specific needs.
The Program involves families and caregivers in the planning process to develop short and long-term care goals.

We have experienced staff and a range of services available to assist people living with disability in Rochester, Elmore and surrounding areas to achieve their goals.
